And to answer Alex's question: Mad-Eye Moody was killed escorting Mundungus Fletcher to the Weasley's. Mundungus was disguised as Harry because they wanted to confuse the death Eaters as to who was the real Harry. Mad-Eye was killed by a Death Eater during this process (I forget which one). And his real name is Alastor Moody.
